ACMPR – How To Become A Legal Medical Cannabis Patient

Legal patients may grow their own plants or designate someone to grow for them. The formula is 1 gram/day = 5 plants (indoors) or 2 plants (outdoors). If your prescription is for 3 grams/day, it would translate to 15 plants (indoors) or 6 plants (outdoors).

To access medical cannabis, or grow your own (or designate someone to grow for you), follow these steps:

1. Download this form and take it to your doctor to fill out Medical Document for the Access to Cannabis for Medical Purposes Regulations , or they may have their own form, OR Choose to register with the Licensed Producer and register with them. Download their form, and have the doctor sign and upload to the LP’s site, or whatever instructions are provided.

2. Your physician must send your medical forms to the Licensed Producer. The medical document can be submitted from the health care practitioner’s office to the licensed producer by secure fax.

3. Once your forms have been received by the Licensed Producer, you may order your product. It will be mailed to your home, (signature required), normally within 24-36 hours, depending on location and Producer.

4. If you wish to grow your own medicine, once you have been approved by a Licensed Producer, download this form from the Health Canada website and send it in to Health Canada.
